Service Coordination (SCS)
Central point of access for persons with a developmental disability in Ottawa, Ontario. Services include:

* Service Access: Provides centralized information, referral to community resources in Ottawa for persons with developmental disabilities and identifies gaps in community resources and services. Maintains a central registry of adults with a developmental disability who request day programme or housing options. Provides support to individuals and their families in choosing options which meet their needs. Referrals are forwarded on a priority-of-need basis when a vacancy occurs.
* Case Coordination: Provides home and community-based support services to families with children of 0 to 18 years of age who have a developmental disability or where a parent is diagnosed with a developmental disability. Serves adults with developmental disabilities living at home or in the community.
* Special Services Worker Bank: Workers, recruited in the community, are linked with families who have a child or adult with special needs. They are employed and paid directly by the family through their Special Services at Home Funding.
