Screening Tool: Depression (in Adults) (2-item Depression Screening Questionnaire)

This survey is designed to provide a quick assessment of whether you might have signs and symptoms related to depression. However, no test is 100% accurate. No matter what your score is, you should seek help if you have any concerns about yourself or your loved ones.

 

The Depression Screening Questionnaire used here is a two-question screening tool recommended by the U.S. Preventive Services Task Force (USPSTF).

1. Over the past two weeks, have you felt down, depressed or hopeless?
2. Over the past two weeks, have you felt little interest or pleasure in doing things?
